3 Risk Register — What Could Bite This Month

Ranked by proximity. Each has a named early-warning and an owner.
🔴 Authorization expiry cliff. ~14 auths expire within 0–7 days (Carelon 0-day cluster + Sunshine). An expired auth = an active client falling out of billing. Elite rule: no auth expires without an active renewal or discharge decision inside 14 days. Owner: Aiman + Sagheer.
🔴 Orphaned rendering provider. Kaylin Flores (exited May 29) still appears on active authorizations 38 days post-exit, and on 5 unreassigned rocks. Billing-integrity and audit risk. Target: zero by 2026-07-14. Owner: Aiman + Ailiasi + Mary.
🟡 Credentialing gates revenue. Marla shows "Request Submitted" (not INN) for Sunshine, Aetna, Cigna — cannot bill under her for those payers yet. Route her cases through a credentialed BCBA for auth/billing until CAQH clears. Owner: Hassan.
🔴 Admission guardrail — 5 closed panels. Aetna Better Health, UHC, Medicare, Humana, Molina are permanently closed for all BCBAs. Do not admit a client whose primary payer is one of these — no billing path exists. Belongs in the intake checklist. Owner: Aiman (intake).

4 Data Integrity — Decisions Need Clean Inputs

An elite clinic runs on one number per metric. A few places currently show two — reconcile before they drive a decision.
Two different "hours" numbers. The Scorecard shows 545 actual RBT hrs (82.6% of the 660 true BEP) while the RethinkBH trend shows 751 insurance-approved delivered hrs (crossing BEP for the first time). Different definitions/denominators. Agree which is the operational hours-of-record so "are we above BEP?" has one answer. Owner: Sagheer + Mary.
Utilization is "Pending." RBT Utilization (target ≥30 hrs/wk/RBT) tells you whether the answer to a new client is "hire" or "load existing RBTs more." Until it's live, capacity decisions are partly blind. Mary's manual Week-1 baseline is the unblock.

Decision 1 — How do we stop free BCBA capacity from sitting idle?
Trinity's 7 open slots vs. 3 unassigned Gate-2 clients is a recurring pattern, not a one-off.
Recommend: adopt a same-day assignment SOP — any client reaching Gate 2 is matched to an available credentialed BCBA within one business day, no waiting for the weekly huddle. Jasmine owns; exceptions escalate.
Decision 2 — Do we open the 4th (and pre-stage the 5th) BCBA req now?
